3/9/2024 EnchantaVaughn Approved Upon arrival to the home setting, Ashiyah became frustrated and began to be disruptive and purposely annoy others in order to draw attention away from herself. Ashiyah failed to utilize appropriate coping strategies and communication skills within the home. Ashiyah chose not to express her feelings without exhibiting inappropriate behaviors at which these behaviors can last up to 20 minutes in duration. QMHP-C actively listened and provided support as mother explained that Ashiyah exhibited difficulty in home and community environments. QMHP-C inquired if Ashiyah was prescribed any medications for her diagnosis. QMHP-C inquired if moving to the new place was the reason behind Ashiyah’s inappropriate behaviors as a way to gain more attention from mom. QMHP-C encouraged mom to talk to Ashiyah about the new changes in a less therapeutic setting to prepare her for transition and dealing with her thoughts and feelings. QMHP-C went through program overview with family to reiterate key points and program objectives that would pertain to their specific situation and helping to build a new therapeutic relationship with the family and Ashiyah without making them feel overwhelmed with questions or being put on the spot. QMHP-C encouraged Ashiyah to continue displaying positive behaviors. QMHP-C supported Ashiyah involvement in selected activities. QMHP-C processed with Ashiyah about what she learned from activity after the review. QMHP-C praised Ashiyah for her cooperation. QMHP-C praised and recognized Ashiyah for her hard work, efforts, participation, and working towards improvement by changing daily habits and implementing a positive and consistent routine. QMHP-C employed positive reinforcement when Ashiyah was able to achieve all of her objectives and encouraged her to maintain her positive behavior by providing her with a review of her objectives. Ashiyah admitted that some of her tension come from not receiving the love and attention she desires by her parents. Ashiyah stated that she wished both her parents were still together because she really missed her dad. Ashiyah informed QMHP-C that she has been learning a lot in each session. Ashiyah thanked QMHP-C for helping her family communicate with each other in a more positive and productive way. Ashiyah stated that talking and sharing things in her life helped her to evaluate her poor choices and correct her mistakes. Ashiyah went on to report that she will practice therapeutic techniques with her family when she feels that it is the right time and place. Ashiyah has attempted to utilize what she acquired from today’s and previous sessions to reduce symptoms of ADHD. Ashiyah responds well to praise and support. Ashiyah is striving for a good report from home every day. Ashiyah wants to make her family proud by doing a good job and showing off her talents and skills. Ashiyah is very independent but can be a team player as well. Ashiyah is observed problem solving on her own and handling her emotions appropriately. Ashiyah responds well to feedback. Ashiyah responds well to the positive reinforcement provided by QMHP-C and is seen increasing self-esteem and confidence. Ashiyah said she was going to practice coping strategies and relaxation techniques in all aspects of her daily living routine in hopes that she will be a better person, daughter, and friend.
